Central Utah Clinics business office is located south of the main campus in Provo at 852 North 500 West, Provo, Utah. We are open Monday through Friday from 9 a.m. to 5 p.m. Our phone number is 801.375.8858.

Central Utah Clinic offers payment plans. Be sure to call one of our representatives if you need this service.

Our billing policies states that interest charges of 1.5 percent accrue each month after the due date. If the bill goes into collection, a 30 percent fee is charged.
